After your treatments are done you will be closely monitored,
in collaboration with your other specialists, through appointments
at the cancer centre or at your regional clinic every 3 to
12 months. You should also see your family doctor for regular
check-ups after your treatments are done and for other health
needs you may have.
The Regional Cancer Centre in Thunder Bay will schedule visits
for you at regional clinics in your home community. The regional
clinic staff will contact either by mail or phone to let you
know the date and time of your next scheduled appointment.
Some of the regional clinics will hold video/teleconferences
with the oncologists in Thunder Bay. The telehealth nurse
(usually a chemotherapy nurse) from the regional clinic will
accompany you to the videoconference and will assist you in
explaining your symptoms or lab work results at that time.
The nurse will explain what will occur during the telehealth
visit and will help you bring your concerns forward to the
oncologist.
Discharge from the Cancer Centre
Once the oncologist determines that your treatment and ongoing
care can be managed by your family physician, he or she will
explain this to you. A dictated consult note will be sent
to your family physician outlining the ongoing follow-up care
required to monitor your condition. This note describes recommended
tests and how often these need to be done. If a change occurs
in your condition, the family doctor can then refer you to
the cancer centre for reassessment.
